K-8 School Enrollment Information

New and returning student enrollment forms for 2023-2024 school year are now available for download.  If you do not have access to printing, please call the school to arrange for a pick-up or if you need to turn in enrollment documents.  We look forward to meeting our new families and welcoming back those who are returning.

Shonto Preparatory Schools accepts enrollment application packets for students who range in age from five (5) years old through fourteen (14) years old. Student must be five (5) years old by December 31 st of the current school year to be eligible for enrollment.  Attendance, behavior, and academic checks will be completed for students who wish to attend SPS.

In addition, we also provide dormitory (residential) for students grades 1st through 8th.  Click here for more information regarding our dorm life and their services.

Thank you for your patience as we navigate through this unique situation.

Returning K-8 students will not need to submit a yearly enrollment application and will automatically roll over.  We ask that parents call in or login to their parent portal with any changes to their household contact information.

Our school nurse will notify parents of pending immunization and will halt student's return until updated.

However, if you are a guardian to a student and your guardianship has expired, please submit a current guardianship/Power of Attorney form.  We do provide our 'School Use' only Power Of Attorney Form

Purpose of the Power of Attorney Form:

You agree to act as the responsible party for the child in school. You will be taking over the duties and obligations the student's parent would normally have for school purposes only. The form gives the school a responsible party (you) to contact with any questions about the child, or to send information home about the child. The school will have the right to ask you questions to make sure you meet the conditions set out in the form.

Parents will need to furnish the following information:

The following are required documents:

  • Enrollment Application;
  • Certificate of Indian Blood (CIB);
  • Official Birth Certificate;
  • Guardian Documentation (if applicable);

Additional documents:

  • Withdrawal from previous school;
